Otis Grant was born in Saint Ann, Jamaica on December 23, 1967. He made his professional debut at the age of 20 years on November 29, 1988 against John Gross. Grant defeated Gross via 4-round unanimous decision, making a successful beginning of his pro career.

Grant got his first shot at a world title at the age of 29 after 29 professional fights. On March 4, 1997 Grant challenged Lonnie Bradley's WBO middleweight title but lost via 12-round split decision.

He became a world champion on December 13, 1997 at the age of 29 years, after by defeating Ryan Rhodes, the then-reining WBO middleweight champion, via 12-round unanimous decision. This was his 32nd fight as a professional.
